Sports Physicals
Central Valley Health offers sports physicals for participants of North Dakota school sanctioned sports. CVHD can bill medical health insurance for the appointment (Sanford Health Plan, BCBS, or Medicaid only) or the cash price is $40. Appointments are required and are available at the main office during business hours.
SPORTS PHYSICALS INCLUDE:
- A one-on-one health history screening
- Physical examination
- A full review of the athlete’s immunization records to ensure they are up-to-date and meet the immunization requirements for their upcoming school year
