Vaccines & Immunizations
North Dakota law states that a child may not be admitted to any public or private school, day care, Head Start program or preschool unless the child’s parent or guardian shows proof that the child has received the required immunizations. If your child has immunization exemptions, please submit an exemption form.
Immunization requirement information is provided below. If you are uncertain of your child’s immunization status, please contact your clinic or CVHD at 252-8130.

As of August 6, 2021 many of the Vaccine Information Sheets (VIS) were updated. The updated VIS can be found on the CDC website https://www.cdc.gov/vaccines/hcp/vis/current-vis.html. Providers may continue to use their current copies until they are depleted. The new VIS should be put into place by February 6, 2022.
Impacted VIS: Hepatitis A, Hib, HPV, Influenza, MMR, MMRV, MCV4, Men B, PCV13, IPV, Tdap, Td, Varicella
Flu shots provide the best protection from seasonal influenza and provide 4 to 6 months of protection from the flu. After receiving the flu shot, it takes about 2 weeks to provide full protection.
Due to COVID, yo must schedule an appointment for a flu vaccination. Visit our “FLU VACCINE” tab. Please bring your insurance card even if you have visited us before.
If you want to save a few minutes at the clinic, complete the forms under ‘Resources’ ahead of time for all patients who will be receiving the flu shot.
We offer the Vaccine for Children (VFC) program. Those children who qualify for VFC must be 18 years of age or younger and Native American, Medicaid eligible or uninsured.
