Injury Prevention
Central Valley Health offers sports physicals for participants of school sanctioned sports.
The cost is $60.00, and is payable by cash, check, or credit card. An appointment is required.
When the student comes to the appointment, they should bring their completed CVHD Sports Physical Worksheet and their completed medical history found on the NDHSAA Sports Physical Form. A parent or guardian must sign the NDHSAA forms if the student is under 18 years old.
Central Valley provides families with safe sleeping resources including a pack ‘n play to ensure families have the resources to keep their child safe through the Cribs for Kids program.
Cribs for Kids ® is a safe-sleep education and distribution program for low-income families to help reduce the risk of injury and death of infants due to unsafe sleep environments. The program addresses specific situations dealing with socioeconomic issues such as crib affordability and cultural practices such as bed sharing and secondhand smoke.
Screening is completed by local partners and eligibility varies by site and is based on availability. Crib Kits for eligible pregnant and newly delivered mothers feature a Graco Pack n’ Play, crib sheet, pacifier and a swaddle sack. Families in need should contact Kim at 701-252-8130.

Central Valley partners each year with local health and safety providers to hold an annual kids safety day event in Jamestown. The event also teaches bike safety with a bike rodeo sponsored by the Jamestown Police Department and provides bike helmets to kids.
Central Valley will also provide bike helmets as needed to daycares or other individuals upon request. The bike helmets are donated by the Optimist Club of Jamestown.
