Sports Physicals

Central Valley Health offers sports physicals for participants of school sanctioned sports.
The cost is $60.00, and is payable by cash, check, or credit card.
Call 701-252-8130 to schedule an appointment.

When the student comes to the appointment, they should bring their completed CVHD Sports Physical Worksheet and their completed medical history found on the NDHSAA Sports Physical Forms. A parent or guardian must sign the NDHSAA forms if the student is under 18 years old.
